How do I make an appointment?
You will require a referral from your GP or Physiotherapist. Once a referral is received then you and your GP will be contacted with an appointment time.
Do I need an x-ray before I come to your appointment?
No you don't. We have x-ray facilities in the building and if required you can usually get an x-ray at the same appointment.
Is there parking at The Collingwood Centre?
Unfortunately there is no patient parking available at The Collingwood Centre. However there is disabled parking and easy drop off. Across the road there is a large public car park and on Collingwood St there is metered car parking also.
What do I need to bring to the appointment?
Most of the information we require has been forwarded to us from your GP of Physio. However to ensure there are no issues, it can be helpful for you to bring any referral notes from your GP or Physio and also your ACC claim number if applicable. Any insurance policy numbers can be sourced after the appointment if required. If you're not sure please just contact Kaye and she will help you through the process.
Also if you have x-rays then these can be useful so please bring them on a disc or in hardcopy (older printed x-rays)
